Late July, 1998. Bodies turning up half-eaten in the forests outside Raccoon City send the Bravo team of the police's elite S.T.A.R.S. unit out to investigate the Arklay Mountains. Their helicopter goes down and they're never heard from again. Alpha team — Jill Valentine, Chris Redfield, Albert Wesker, and Barry Burton among them — flies out looking for survivors and is ambushed by a pack of mutated, feral dogs. Pilot Brad Vickers panics and abandons the ground team, who flee into the only shelter in sight: an isolated mansion belonging to the reclusive Spencer family. One member of the group goes missing before the doors even close behind them.

What looks empty at first is anything but. The team splits up to search room by room, and finds a building riddled with traps, locked vaults, and the shambling remains of the mansion's staff — zombies infected by a virus that escaped Umbrella's control. Diaries and research notes scattered through the estate spell out just how far the experiments went, and a grim discovery makes it worse: a slain zombie left to rot doesn't stay dead. Given enough time, it rises again as a Crimson Head — faster, stronger, and far harder to put down than it ever was the first time.

Deeper still is a presence even the zombies fear: Lisa Trevor, a shackled, disfigured figure who can't be killed by any weapon in the mansion. Diaries left by architect George Trevor reveal the truth — Umbrella imprisoned his entire family the moment his design work on the mansion was finished, to keep its secrets buried. His daughter Lisa became a living test subject for decades of viral experimentation, her body twisted and regenerating, her mind fractured into fragments of a life before the mansion. All she wants is to find her mother again — not knowing that all that's left of her is bones sealed in a crypt beneath the estate.

The truth about the mission surfaces in the mansion's hidden underground laboratory: Wesker was never really S.T.A.R.S. at all. He orchestrated the entire operation on Umbrella's orders, planting the team here to observe the T-Virus firsthand — and coercing Barry into silence and sabotage by threatening his family the whole time. When he's finally cornered, Wesker unveils his ultimate creation: the Tyrant, a bioweapon engineered to eliminate anyone who gets too close to the truth. It turns on him first, mauling its own creator before the survivors can even process what's happening.

With the lab's self-destruct sequence counting down, the survivors race for the surface. On the helipad, the Tyrant bursts out for one final fight, mutated further by direct exposure to the Virus. A rocket from the launcher Brad drops at the last possible second finally puts it down, moments before the whole estate goes up in flames below. As the survivors' helicopter pulls away, Umbrella's cover-up machine is already starting to turn — and Raccoon City's reckoning is still years away.
