Items

Snake's survival gear: camouflage, medical supplies, night vision and thermal goggles, detectors, and every gadget EVA hands him along the way.

Camouflage

Camouflage Uniforms

A wardrobe of jungle, rock, water, and snow patterns. Matching your uniform to the current biome is what actually keeps you off enemy radar — more than crouching or crawling alone.

Location
Unlocked and picked up across the map as you move through different biomes
Effects
  • Alters Snake's camo index depending on the surrounding terrain
  • Switching uniforms constantly is the single biggest factor in staying undetected

Face Paint

The uniform's companion piece. Each pattern favors a different kind of terrain — pick the one that matches your current surroundings.

Location
Scattered across the map, and awarded for defeating certain bosses without lethal ammo
Effects
  • Camouflages Snake’s face to match a specific environment (Woodland, Water, Snake, Desert...)
  • Stacks with the uniform to maximize your camo index

Medical

First Aid Kit (Bandage, Disinfectant, Splint, Styptic) & Life Medicine

Snake's wound system treats every injury separately instead of a single health bar refill — skipping treatment costs you max HP for the rest of the mission.

Location
Medical kits scattered across bases and camps throughout the map
Effects
  • Each wound type needs its own treatment — disinfect cuts/burns, splint fractures, use styptic to stop bleeding before bandaging
  • Life Medicine restores health directly, like classic Metal Gear rations
  • Ignoring a wound permanently lowers your maximum health

Fake Death Pill & Revival Pill

A potassium-cyanide pill that simulates death to fool a boss or make an emergency escape, paired with the pill that reverses it before the "false death" becomes real.

Location
Handed over by EVA together with the rest of the medical kit during Operation Snake Eater
Effects
  • The Fake Death Pill fools every boss except The Boss into believing Snake is dead
  • The Revival Pill wakes Snake back up — waiting too long turns the Game Over into a Time Paradox
  • Taking the Revival Pill is mandatory to wake from The Sorrow encounter in Chapter 5

Field Equipment

Night Vision Goggles

Essential for the underground caves if you don't want to rely on the torch. Amplifies whatever low light is available instead of detecting heat.

Location
In the Chyornaya Peschera cave branch, northwest of the waterfall at the end of the crawlspace maze
Effects
  • Illuminates completely dark environments by amplifying ambient light
  • Unlike the thermal goggles, can't detect traps, camouflaged enemies, or animals

Thermal Goggles

Detects heat signatures rather than amplifying light — the direct counter to any boss relying on optical camouflage.

Location
A locker in Rassvet, the southwest corner of the Ponizovje Warehouse, or the Weapons Lab East Wing in Groznyj Grad
Effects
  • Highlights body heat through camouflage and dense foliage
  • Also detects mines and hidden food, despite producing no real heat
  • Vital against The Fear and for tracking The End

Directional Microphone

A parabolic mic for picking out sound at long range — the tool that turns The End's hunting duel from guesswork into tracking.

Location
Recovered from the tracking gear before entering the Graniny Gorki forest
Effects
  • Amplifies distant sound, including a hidden target's breathing
  • Essential for locating The End in the Svyatogornyj forest

Mine Detector

A dedicated sensor for spotting Claymores before you walk into one — crucial through Bolshaya Past's minefields.

Location
Hidden in the military depots of Bolshaya Past
Effects
  • Beeps as you approach Claymore mines, at the cost of battery power

Motion Detector & AP Sensor

Two different ways of knowing something is near you before you can see it — one precise, one just a warning.

Location
Military depots in Bolshaya Past and Groznyj Grad
Effects
  • Motion Detector marks the on-screen position of nearby guards and animals
  • AP (Anti-Personnel) Sensor uses heartbeats to indicate enemy proximity without a precise location
  • Useful together since this game has no Soliton Radar

Active Sonar

A submarine-style sonar ping. Great for planning a route through a heavily patrolled area, at the cost of alerting anyone nearby.

Location
Unlockable by fully exploring the Groznyj Grad base
Effects
  • Emits a pulse that briefly reveals every enemy in the area on your display
  • Gives away your own position when used

Binoculars

Nitrogen-filled to avoid fogging. The first tool to reach for before committing to a route through open ground.

Location
Default starting inventory
Effects
  • Adjustable zoom for scouting routes and patrol patterns from a safe distance
  • No thermal imaging — switch to the thermal goggles to spot traps

Compass

A navigation aid added in this remake to make orienting yourself in the open jungle areas easier.

Location
Default starting inventory
Effects
  • Shows Snake’s current position and facing direction on screen

Tools & Consumables

Cardboard Boxes (A, B, C)

The classic stealth disguise. Each of the three boxes has its own truck route wired to it, turning it into both cover and a shortcut.

Location
Box A: on top of crates in Rassvet or the southwest corner of the Ponizovje Warehouse | Box B: at the end of the tunnel behind the waterfall in Tikhogornyj | Box C: the alley southeast of the Groznyj Grad prison
Effects
  • Hides Snake in plain sight while stationary
  • Riding in the back of a truck while wearing one fast-travels you to that box’s linked destination
  • Box C is most useful right after escaping your prison cell

Cigar

A small trade-off item: better sniping stability at the cost of health and stealth if you're not careful about where you light it.

Location
Various soldier camps across the map
Effects
  • Steadies Snake’s aim while sniping
  • Can double as an emergency light source in the dark
  • Slowly drains health while lit, and can draw attention outdoors at night

Bug Juice

A simple repellent that keeps the jungle's wildlife from becoming a second enemy on top of the actual patrols.

Location
Found near swamp and jungle areas
Effects
  • Repels insects and leeches from attaching to or biting Snake

Croc Cap

Collectible

A crocodile-shaped cap. As unlikely as it sounds, wearing it keeps every guard dog in the game from noticing you.

Location
Across a tightrope over a swamp, shortly after reuniting with EVA and defeating the Ocelot Unit
Effects
  • Wards off guard dogs entirely while equipped

Camera

Collectible

Doesn't unlock anything essential to the story — purely a photo mode reward for finishing the campaign.

Location
Automatically obtained after completing the game once
Effects
  • Lets you take first-person photographs for your own collection

Monkey Mask

Collectible

An alternate way to earn permanent immunity to guard dogs, tied to a separate minigame rather than a story pickup.

Location
Awarded for clearing all seven stages of the Snake vs. Monkey minigame
Effects
  • Guard dogs no longer attack Snake while equipped

Key Items

Keys A, B & C

Three single-use story keys, each handed to you at a specific point rather than found lying around.

Location
Key A: given by Granin at the Graniny Gorki labs | Key B: given by EVA near the top of the mountain ruins | Key C: given by EVA under the waterfall
Effects
  • Key A opens the warehouse door leading to the research labs
  • Key B opens the locked door in the mountains below the ruins
  • Key C opens the Shagohod engineering room
