Late September 1998. Umbrella's cover-up of the Arklay Mountain incident collapses as the T-Virus leaks into the municipal water supply, turning the entire population of Raccoon City into undead cannibals.

Leon S. Kennedy, a rookie police officer driving in for his first day on the force, and Claire Redfield, a college student searching for her missing brother Chris, arrive in the city right as the nightmare reaches its peak. After meeting in a diner and surviving a collision caused by a blazing tanker truck, a wall of fire forces them down separate paths: in the canonical storyline (Claire A / Leon B), Claire enters through the front gates of the R.P.D. precinct, while Leon finds entry via the rear alleyway and helipad.

Inside the ruined precinct, the full scale of the conspiracy begins to unravel. Umbrella lead scientist Dr. William Birkin had finalized an even deadlier pathogen: the G-Virus, capable of continuous cellular mutation and evolution. When an Umbrella Special Forces team (U.S.S., led by HUNK) ambushed Birkin to seize the virus, he injected himself with a sample as he lay dying, transforming into an unstoppable biological monster (G-1) that slaughtered the squad and shattered the viral vials into the sewers.

In a precinct deliberately sabotaged by corrupt Police Chief Brian Irons, Claire discovers and protects young Sherry Birkin, who fled through the building carrying a pendant containing a pure G-Virus sample hidden by her father. Simultaneously, Leon crosses paths with Ada Wong, an enigmatic woman claiming to look for her boyfriend John, who is secretly an undercover corporate spy tasked by a rival firm with securing the G-Virus.

Driven by the G-Virus's biological imperative to reproduce only within genetically compatible biological relatives, William Birkin stalks Sherry through the underground channels and implants a G-embryo inside her. Claire is alerted by Annette Birkin (Sherry's mother and William's co-researcher) about the fatal nature of the embryo, sparking a desperate race against time to synthesize a cure before the parasite hatches.

Meanwhile, Umbrella deploys the Tyrant T-103 (Mr. X) via airdrop — an unrelenting bio-weapon assigned to retrieve the viral pendant and eliminate all surviving witnesses. Leon faces non-stop harassment from Mr. X throughout the precinct and sewer waterways. During a brutal ambush, Ada shields Leon from the Tyrant, sustaining catastrophic injuries and seemingly dying in his arms.

Deep inside Umbrella's Underground Laboratory, Claire successfully synthesizes the DEVIL vaccine at the chemical terminals and administers it to Sherry, neutralizing the viral embryo just in time. Annette dies by her daughter's side after asking for forgiveness, while William Birkin mutates into colossal predatory forms (G-3 and G-4).

With the facility's self-destruct sequence activated, Leon reaches the underground cargo freight elevator and is cornered by the uninhibited Super Tyrant. At the climax of the battle, a shadowed figure — Ada Wong, having survived — tosses Leon a heavy rocket launcher, allowing him to obliterate the Tyrant.

On the emergency evacuation train, an enormous amorphous mass of biomass (Birkin G-5) engulfs the rear cars. Leon and Claire unleash their full arsenal to destroy the monstrosity seconds before the train detaches and escapes the underground facility's destruction. Leon, Claire, and Sherry emerge together into the dawn, committed to bringing Umbrella down for good.
