Late July 1998. S.T.A.R.S. Bravo team is dispatched into the Arklay Mountains to investigate a string of grisly murders — the same case that will soon pull Alpha team into the Spencer Mansion. Their helicopter develops engine trouble and is forced down near a stretch of railway. Medic Rebecca Chambers, the youngest member of Bravo team, splits off to investigate gunfire and finds an abandoned commuter train, the Ecliptic Express, stopped dead on the tracks.

Boarding it, she discovers she isn't alone: Billy Coen, a former Marine lieutenant, is being transported to execution after a court-martial convicted him of murdering 23 civilians. Billy insists he was framed. Before Rebecca can make sense of any of it, the train's real cargo — the walking dead — corners her, and survival forces an uneasy alliance with the man she's supposed to be afraid of.

Fighting through the train reveals it was never just a train — it's a mobile extension of a nearby Umbrella Training Facility, a compound built to condition new recruits and, as Rebecca and Billy dig deeper, to conceal decades-old secrets. Passwords, statues, and puzzle-locked doors guard a research wing that predates the T-Virus outbreak everyone assumes started this crisis.

The deeper the two go, the clearer it becomes that this facility isn't Umbrella's newest project — it's one of its oldest, built around a man the company would rather everyone forgot.

Beneath the facility, Rebecca and Billy uncover the truth: Dr. James Marcus, one of Umbrella's three founders and the scientist who first isolated the Progenitor Virus, was assassinated decades earlier on orders from his own partners — Oswell Spencer and Edward Ashford — once his research into leeches became a threat to their control of the company. His killer that day was a young Albert Wesker, then a rising Umbrella agent. Marcus should have died. Instead, the leeches he'd spent his life studying kept his consciousness alive, fused with their mass, and let him rebuild a body from their collective will.

For years afterward, the reanimated Marcus continued his research from hiding, breeding a strain of intelligent, blood-hungry leeches and grafting his own genetic material onto Umbrella's early Tyrant program — creating the lumbering, half-finished Prototype Tyrant that stalks Rebecca and Billy through the lower levels. His endgame is simpler than any of it: use the facility's self-destruct systems and his monstrous new form, the Queen Leech, to make sure Umbrella burns along with him.

With the facility's countdown running and Marcus's final form closing in, Rebecca and Billy fight their way to the surface, destroying the Queen Leech and the last of Marcus's decades-long grudge in the process. As the compound collapses behind them, Billy chooses to vanish rather than face a legal system that already condemned him once — Rebecca reports him dead, the closest thing to a pardon she can offer.

Rebecca herself has no time to process what she's just survived. She has a train wreck to explain, a Bravo team to reunite with, and no idea yet that the worst is still ahead of her: an Alpha team helicopter is inbound to the same mountains, carrying Jill Valentine and Chris Redfield toward the mansion that will make Umbrella's crimes public.
