The town's regulars and its bosses — Grey Children, Nurses, Air Screamers, and the things waiting at the end of each chapter.

Grey Children

Encounter

Small, faceless humanoids that swarm in packs and slash with tiny knives. Individually harmless, but they rush you in groups — back into a doorway or hallway so only one can reach you at a time.

Recommendations
  • Shotgun at close range if more than one shows up
  • Handgun headshots work fine one-on-one

Mumblers

Encounter

Shambling, barely-human wretches that groan and stagger toward you. Slow enough to avoid entirely in open areas — not every fight in this game is worth having.

Streets, Sewers, Alchemilla Hospital
Recommendations
  • Handgun to the head
  • Easy to outrun if you just need to pass through

Nurses

Encounter

Blood-soaked hospital staff that shuffle unnervingly before lunging without warning. Fragile once you commit to a hit, but their jerky movement makes it easy to misjudge the timing.

Alchemilla Hospital (Otherworld)
Recommendations
  • Kitchen Knife or Flauros — they go down fast in melee
  • Watch the twitching; it telegraphs their lunge

Romper

Encounter

Squat, clawed creatures that hunt in small packs and hit far harder than their size suggests. Don't get surrounded — thin them out one at a time from a doorway when you can.

Lakeside Amusement Park, Old Silent Hill
Recommendations
  • Shotgun up close
  • Keep moving — they hit hard if they close in

Air Screamers

Encounter

Bat-like flyers that dive-bomb and shriek overhead. Annoying more than dangerous; unless you're stocked on rifle ammo, it's usually faster to just push past them.

Old Silent Hill, Resort Area, Nowhere
Recommendations
  • Hunting Rifle if you have the ammo to spare
  • Otherwise just run — they rarely deal serious damage

Bosses

Split Head

Boss · Medium

A fast, snapping creature fought in a burning hallway. Keep the fire between you and it while circling, then watch for the moment it opens its mouth wide — that's your window to land a clean hit and force it to retreat.

Midwich Elementary School — Boiler Room hallway (Otherworld)
Recommendations
  • Shotgun for guaranteed damage
  • Circle the bonfire in the room and only stop to fire when it is not lunging

Twin Feeler (Larva Form)

Boss · Medium

A giant larval worm that burrows underground between attacks and spits poison. It flees rather than dying outright — expect a second, tougher fight once it's had time to change.

Shopping Mall, 2F (Otherworld)
Recommendations
  • Hunting Rifle, picked up mid-fight near a broken storefront
  • Stand on the metal grating on the floor — it cannot burrow under you there

Floatstinger (Moth Form)

Boss · Hard

What the Twin Feeler becomes after fleeing the mall — now airborne, tankier, and attacking with both a stinger lunge and a ranged poison spit. A slow fight; keep your distance and don't panic- fire, it can absorb a lot of punishment.

Outside Alchemilla Hospital
Recommendations
  • Finish your Rifle ammo, then switch to the Shotgun
  • Keep moving in one direction and do not stop to admire the poison spit

Cybil (Corrupted)

Boss · Hard

If Cybil's own fear and the town's influence get the better of her, she turns on Harry in the final stretch. A tragic, optional fight — exactly when and whether it triggers depends on earlier events, so treat this entry as a guide rather than a guarantee.

Location
Nowhere, late game — only if Cybil has not been kept calm/safe through the story
Recommendations
  • Handgun, aiming to disable rather than kill
  • This encounter is avoidable depending on your choices earlier in the game

The Incubus (Final Boss)

Boss · Deadly

The being the Order has spent the whole game trying to birth through Alessa. Its exact shape and the difficulty of the fight shift depending on choices made earlier in the playthrough — come in with every scrap of ammo you've been saving.

The ritual site, beneath Nowhere
Recommendations
  • Save your Rifle and Shotgun ammo specifically for this fight
  • Whether you've collected the items to weaken it changes how this fight plays out
