Lakeview Hotel — The Otherworld
You're in the final stretch. Everything has changed, and — for a change — your flashlight doesn't work anymore.
Save by the door to Room 313, then head down to the second floor. Enter Room 202, which automatically transports you to Room 219. From there, take the elevator down to Basement 1.

The Flooded Basement
It's flooded down here, so watch your step. Head to the Kitchen (supplies inside, if you want them), then cross it to the hallway and reach the employee stairwell behind the blue door.
A scene plays out here, and the stairs end up on fire — no going up. Backtrack the way you came, but you'll find yourself somewhere different than expected.

Reaching the Last Save Point
Head up the stairs and take the only available door (also blue). Follow this hallway to a door that leads somewhere well outside the hotel's normal map.
You'll be in another corridor — watch for enemies here — and simply need to keep going until you spot nine red squares ahead: the final save point.

Before the Final Stretch
You're in the antechamber to the endgame. Save, double-check your inventory, and take a breath before crossing the double doors ahead.

Boss Fight: The Twin Pyramid Heads
The Twin Pyramid Heads
Boss · HardTwo Pyramid Heads greet you before the final confrontation, in a room with enough space to actually work with. Since they share health, focus fire on whichever is closer and keep your strongest ranged weapon loaded.
Attacks- Heavy melee lunges from two separate Pyramid Heads at once
Strategy- They share one health pool — damage to either counts
- Run corner to corner in this larger room, firing at safe distance rather than trading hits
Once they turn to statues, take one item from each: a Scarlet Egg and an Ochre-Colored Egg. Place the Scarlet Egg on the left door and the Ochre Egg on the right — both open, and it doesn't matter which you take.

The Hallway and Mary's Voice
Cross through into a lobby, then through its only door into a long hallway.
Mary's Message

Bed or Window
On the other side, head left (from James's perspective) up a metal staircase. At the top, approach either the bed or the window — which one is available depends on your choices throughout the playthrough. Doing so triggers the scene leading straight into Silent Hill 2's final confrontation.

Final Boss: Mary
Mary
Boss · HardNot a mechanically complicated fight, despite everything it's built up to. If you've hoarded Hunting Rifle ammo, this is the moment to spend it — she stays suspended in the air for most of the fight, and both her whip lunge and moth swarm have more range than you'd expect, so keep your distance. Once she falls, move in to finish it.
Attacks- A whip-like lunge with surprising reach
- A swarm of moths that homes in on you
Strategy- Hunting Rifle ammo, saved specifically for this fight
- Avoid the Shotgun — both attacks outrange it
- Finish her off once she drops to the ground
