Waking Up in the Otherworld
After a disorienting scene where James seems to black out and hallucinate, you're back in control — now inside the hospital's twisted Otherworld version.

Second and Third Floor
Cross the double doors ahead to save, then head to the elevator to go up to the second floor (Room C2 is worth a stop for supplies on the way). Explore a little, then head to Room M6 for a Dry Battery and the Basement Storage Room Key.
Take the elevator to the third floor and find the stairs down to the basement. Move a piece of furniture inside to trigger a new scene, then head down the stairs for the Copper Ring.

Two Rings and a Box Puzzle
Head back up to the third floor, then take the elevator down to the second. After a radio message, head to the Day Room and check the fridge for the Lead Ring.
On the same floor, the Store Room holds a locked box with a button puzzle across three rows:
- Row 1: third button.
- Row 2: first button.
- Row 3: third button.
Head to the room next to the elevator and use both rings there — this opens a path down a staircase, continuing along the only route available.

Leaving the Hospital
After a scare, you'll reach the first floor. Head to the Director's Room for the Waiting Room Key, sitting on a desk. A new scene follows, and the key finally lets you leave the hospital behind.

South Vale — The Alternate Streets
Outside, the streets of Silent Hill have gone considerably darker — good thing you have the flashlight. Head south on Carroll Street, then turn onto Rendell Street. Continue toward Saul Street and use the map to find a door leading to the Gonzalez Mexican Restaurant.
From there, head to Lindsey Street — the second building's entryway holds a Wrench. Continue north on Lindsey, over to Katz Street, until you find a door marked with a message written in blood. Go through it, then head to Rosewater Park via Munson.

Digging Up the Bronze Key
Rosewater ParkBronze KeyFind a statue in the park and check the patch of dirt behind it — use the Wrench to dig, and you'll uncover a box containing the Bronze Key. With it, head to the Silent Hill Historical Society, marked on the map.

Silent Hill Historical Society
A long stretch, but good news: no boss fight this time.
Head down a long staircase and through the door at the bottom. Cross the room beyond and exit through its only door, then turn left, then right, to another door. Inside is a large hole in the floor — drop down (yes, on purpose).
You'll land in a kind of well with a way out: check the walls for a section that looks different, then hit it with a melee weapon to reveal a hidden door. This leads into Silent Hill's sewers.

The Sewers and the Roach Room
Head right (the other way is a dead end) until you reach two doors — take the right one, then in the next room, right again, for a Key with a Spiral Engraving.
Right on cue, your flashlight runs out of batteries — good thing you're carrying that Dry Battery. Swap it in and get ready: the next room is crawling with roaches. Push through to the door on the far side, where a keypad panel blocks your way.
Cracking the Panel CodeYou don't have the code written down anywhere, but you don't need it — the panel itself lights up the digits that belong in the combination (in a different color from the rest), and the code is always three digits.
- 3 lit digits: no repeats — only 6 possible orderings to try.
- 2 lit digits: one digit repeats — still only 6 possibilities.
Try the door after each guess; there's no sound or visual cue telling you when you've gotten it right.
Once you crack it, you can leave the roach-infested room behind. Follow the hallway beyond to a side room, where a grate in the floor opens with the Key with a Spiral Engraving. Drop through it — that's the end of the chapter, and you've arrived at Toluca Prison.
